How to Take Your Blood Pressure at Home Correctly

The single biggest source of home blood pressure error isn't technique. It's a cuff that doesn't fit, and most people have no idea theirs is wrong.

Quick answer: An undersized cuff can inflate your reading by nearly 20 mmHg; an oversized cuff can lower it by about 4 mmHg. Measure your arm circumference before buying a monitor, choose a validated upper-arm device (never wrist or finger), and take two readings a minute apart, morning and evening, for about a week to get a number you can actually trust.
Why cuff size matters more than almost anything else
A 2024 study covered by the American Heart Association found that a cuff too small for the arm it's wrapped around produced readings nearly 20 mmHg too high, while a cuff too large produced readings about 4 mmHg too low. That's not a rounding error. A 20-point overestimate can turn a normal reading into an apparent stage 2 hypertension diagnosis, and a 4-point underestimate can hide a real problem.
Despite this, most people never measure their arm before buying a monitor. They guess based on the box, or they buy whatever their pharmacy has in stock. Given how much a mismatched cuff can distort the number, this is the first thing to fix, before technique, before timing, before anything else.
How to actually measure your arm
Wrap a soft tape measure around your bare upper arm at the midpoint between your shoulder point (acromion) and elbow point (olecranon). Keep the tape snug but not tight, and note the measurement in centimeters.
The American Heart Association recommends a cuff bladder that covers roughly 80 to 100 percent of your arm circumference in length and about 40 percent in width. If your measurement falls right on the boundary between two sizes, choose the larger one. An undersized cuff is a far more common and more dangerous error than an oversized one.
Cuff sizing is not standardized across brands. A "large" cuff from one manufacturer can cover a different range than a "large" cuff from another. Don't assume a size label means the same thing on every device; check the specific centimeter range printed on the cuff or in the manual.
Choosing the right device
Use an automatic, upper-arm cuff-style monitor. Wrist and finger devices are not recommended because they are less accurate. Clinical guidance does not currently support cuffless devices like smartwatches for diagnosing or managing high blood pressure, regardless of what the marketing implies.
"FDA cleared" is not the same as "accurate." The measurement protocol that actually produces a usable number
Avoid caffeine, exercise, and smoking for at least 30 minutes beforehand.
Empty your bladder first.
Sit in a chair with back support and feet flat on the floor. Don't cross your legs.
Rest your bare arm on a flat surface at heart level, palm up, muscles relaxed.
Sit quietly for five minutes before the first reading. Don't talk during measurement.
Take two readings about one minute apart, and record both rather than only the better one.
How many readings do you actually need
One reading, taken once, tells you almost nothing reliable. Clinicians who specialize in this note that roughly a dozen properly taken readings, on a validated device with the correct cuff size, spread across one to two weeks, are generally enough to determine whether someone has hypertension or needs a medication adjustment.
A practical approach: measure twice in the morning and twice in the evening for seven days, record every value, and bring the full log to your clinician rather than a single favorable number. The pattern matters more than any individual reading, including the best one you happen to get.
Mistakes that quietly wreck a reading
Measuring over clothing instead of a bare arm.
Letting the arm hang unsupported at your side instead of resting at heart level.
Crossing your legs, which can meaningfully raise the reading.
Talking or being talked to during the measurement.
Measuring immediately after climbing stairs, an argument, or a stressful phone call.
Using a device that has never been checked against an office reading for agreement.
When to recheck your device's accuracy
The American Heart Association recommends having your device checked against an office measurement about once a year. Bring your home monitor to an appointment, take a reading with it right after the clinical staff takes theirs, and compare. A meaningful, consistent gap between the two means the device or your technique needs attention.

Are wrist blood pressure monitors accurate?
