Quick answer: Triglycerides are fats carried in the blood. A result below 150 mg/dL is generally considered healthy; 150 through 199 is borderline high, 200 through 499 is high, and 500 or higher is very high. Current guidance treats 500 mg/dL as a threshold for prompt evaluation of secondary causes and pancreatitis prevention, with particular concern at 1,000 mg/dL or higher. One result is the beginning of interpretation, not the entire diagnosis.
What triglycerides are and why they matter
Triglycerides are the body's main storage form of fat. After a meal, the intestine packages dietary fat into particles called chylomicrons. The liver can also make triglycerides, especially when energy intake exceeds immediate needs. These fats travel through the bloodstream so tissues can use or store them.
Triglycerides are not the same thing as cholesterol. Both travel inside lipoprotein particles, but they serve different roles. That is why a person can have high triglycerides while total cholesterol or calculated LDL appears acceptable. The full lipid pattern, medical history, and reason for testing all matter.
Persistently elevated triglycerides can mark a greater burden of triglyceride-rich remnant particles and cardiometabolic risk. At much higher levels, the immediate concern shifts toward chylomicronemia and acute pancreatitis. The same laboratory name therefore covers more than one clinical problem.
How to read the common ranges
Below 150 mg/dL: generally considered a healthy triglyceride level for adults.
150 through 199 mg/dL: borderline high.
200 through 499 mg/dL: high.
500 mg/dL or higher: very high and a threshold for prompt evaluation of pancreatitis risk.
1,000 mg/dL or higher: especially concerning because chylomicrons often accumulate and pancreatitis risk rises.
These categories help organize the conversation, but they do not create a complete treatment plan. Fasting status, repeat measurements, diabetes, liver or kidney disease, thyroid function, alcohol use, pregnancy, medicines, family history, and overall cardiovascular risk can all change what happens next.
A severe result is not a do-it-yourself project: If a fasting result is 500 mg/dL or higher, contact the clinician who ordered it promptly. At 1,000 mg/dL or higher, timely medical management is especially important. Severe upper abdominal pain with nausea or vomiting can be a sign of pancreatitis and warrants urgent assessment.
Why triglycerides become elevated
A high result is often produced by several factors at once. Insulin resistance and poorly controlled diabetes can increase the flow of fatty acids to the liver and increase production of triglyceride-rich particles. Excess alcohol and high-glycemic foods can also push triglycerides upward in susceptible people. Weight gain, central adiposity, and low activity often travel with the same pattern.
Secondary medical causes include chronic kidney disease, nephrotic syndrome, hypothyroidism, pregnancy, chronic inflammatory conditions, and some liver disorders. Medicines can contribute too. The ACC pathway lists examples that include certain beta blockers, diuretics, steroids, estrogens, tamoxifen, immunosuppressive drugs, and some HIV therapies. Never stop a medicine based on a list. Ask the prescriber to review the complete regimen.
Genetics can set the background. Some people inherit a greater tendency to produce or clear triglyceride-rich particles poorly. Very high levels may reflect a genetic vulnerability amplified by diabetes, alcohol, medicines, or diet. Far less commonly, a single-gene disorder is the primary driver. Family history and the pattern across repeated tests help guide that distinction.
Fasting versus nonfasting testing
Routine lipid screening does not always require fasting. Expert consensus recognizes that nonfasting testing is practical and informative, and average triglyceride changes after usual meals are modest at the population level. A nonfasting value of 175 mg/dL or higher is commonly treated as elevated in current guidance.
Fasting becomes more useful when the result is high enough to require clarification, when a clinician is evaluating metabolic syndrome or a possible familial lipid disorder, when the response to treatment needs a consistent comparison, or when pancreatitis risk is in view. The ACC pathway uses fasting measurements to confirm persistent hypertriglyceridemia and recommends at least two measurements separated by at least two weeks before risk-based nonstatin decisions.
The correct question is not whether fasting is always better. It is whether the testing conditions answer the clinical question. Record the fasting interval, recent alcohol intake, illness, unusual meals, and medication changes so the repeat test has usable context.
Why the number can move quickly
Triglycerides can change more from day to day than LDL cholesterol. A recent meal, alcohol, glycemic control, acute illness, weight change, sleep, medicines, and normal biological variation can all move the result. Small studies have also documented within-person daytime and circadian variation.
That responsiveness is useful, but it can tempt people to overinterpret one dramatic week. ACC defines persistent hypertriglyceridemia after four to twelve weeks of lifestyle work, a stable statin dose when indicated, and management of secondary causes. It also uses repeated fasting values. The goal is to distinguish a temporary spike from a durable pattern.
A lower repeat value is encouraging, but it does not prove that a single food, supplement, or hack caused the change. Compare like with like and judge trends alongside the rest of the lipid panel and medical plan.
How to read the rest of the lipid panel
HDL cholesterol is often lower when triglycerides are elevated, particularly in insulin resistance. Non-HDL cholesterol subtracts HDL from total cholesterol and represents cholesterol carried in all potentially atherogenic particles. ApoB counts atherogenic particles more directly and can be useful in selected people with high triglycerides or cardiometabolic risk.
LDL deserves special care. Many reports calculate it using an equation. When triglycerides are markedly elevated, that estimate can become unreliable, and above 400 mg/dL common equations may not perform adequately. A clinician may emphasize non-HDL cholesterol, request ApoB, use a direct LDL method, or repeat the panel after triglycerides improve.
None of these measures is a replacement for the others. The purpose is to match the measurement to the decision: pancreatitis prevention, atherosclerotic cardiovascular risk, treatment response, or investigation of an inherited disorder.
How treatment priorities change with severity
Lifestyle and correction of secondary causes are the foundation at every level. The exact plan may include improving glucose control, changing alcohol exposure, adjusting food quality and energy balance, increasing appropriate physical activity, addressing sleep, and reviewing medicines. Severe hypertriglyceridemia requires a more urgent and often more specialized plan than a mild elevation.
For cardiovascular risk, the 2026 ACC/AHA guideline keeps statin therapy as the foundation when medication is indicated. Selected high-risk adults with managed LDL cholesterol and triglycerides in the 150 through 499 mg/dL range may be candidates for prescription icosapent ethyl. That evidence comes from specific populations and a prescription product. It cannot be generalized to every person or to over-the-counter fish oil.
At 500 mg/dL or higher, clinicians evaluate secondary causes and may consider triglyceride-lowering medication to reduce pancreatitis risk. At 1,000 mg/dL or higher, the plan may include an intensive, clinician-directed dietary approach and medication. Dose, drug choice, interactions, pregnancy, kidney function, and liver function all require professional review.
The founder lesson: urgency without panic

Can high triglycerides cause symptoms?
Usually not. Many people learn about them from a blood test. Extremely high levels can accompany eruptive skin lesions, eye findings, or pancreatitis, but absence of symptoms does not make a severe value safe.
Is high triglycerides the same as high cholesterol?
No. Triglycerides and cholesterol are different lipids. They share transport particles, so the full lipid panel and overall risk profile should be interpreted together.
Can I lower triglycerides in a few days?
The number can move quickly, particularly when a major driver changes, but a short-term drop does not establish a stable trend. Persistent status is usually judged with repeat measurements and a longer clinical window.
Does a normal A1C rule out a triglyceride problem?
No. A1C reflects average glucose over roughly two to three months, while triglycerides reflect lipid transport and can change faster. A normal A1C does not rule out insulin resistance, alcohol effects, genetics, thyroid disease, liver disease, medicines, or other causes.
Is the triglyceride-to-HDL ratio a diagnosis?
No. It can be a useful pattern or research marker, but there is no universal cutoff across sex, ethnicity, age, clinical setting, and unit system. It should not replace established testing.
Is over-the-counter fish oil the same as prescription therapy?
No. Prescription omega-3 products have standardized ingredients and doses and are evaluated for specific uses. Evidence for prescription icosapent ethyl should not be transferred to ordinary supplements.
