Each value describes a different part of lipid transport.
A standard lipid panel reports total cholesterol, LDL cholesterol, HDL cholesterol, and triglycerides. Triglycerides are not cholesterol, even though they appear on the same report.

What the calculator adds
Two subtraction measures reveal what the standard lines do not show directly.
Non-HDL-C represents cholesterol outside HDL, including LDL and other atherogenic apoB-containing lipoproteins. It can be calculated from fasting or nonfasting total cholesterol and HDL-C.
This is equivalent to non-HDL-C minus LDL-C. It is a research estimate of cholesterol in triglyceride-rich remnant particles, not a direct laboratory measurement. It inherits error from the LDL method and the other panel measurements.
Ratio math
Three ratios, four displays
One ratio changes when the unit convention changes.
Those two ratios remain the same in mg/dL and mmol/L because all cholesterol components use the same conversion factor.
TG/HDL is different. Triglycerides and cholesterol have different molecular conversion factors. A TG/HDL ratio of 3.00 using mg/dL values is about 1.31 using mmol/L values. A cutoff from a study cannot be used unless you know which unit convention the researchers used.
LDL method
Calculated does not mean measured
The equation printed behind your LDL result matters.
Many laboratories estimate LDL-C from total cholesterol, HDL-C, and triglycerides. The older Friedewald equation uses triglycerides divided by 5 in mg/dL and is not used when triglycerides are 400 mg/dL or higher. Newer Martin-Hopkins and Sampson-NIH equations improve accuracy in many situations, but they remain estimates.
A July 2026 Circulation review states that either Martin-Hopkins or Sampson should be used and that Friedewald is less accurate. CDC NHANES documentation publishes all three calculations and limits the NIH Equation 2 calculation to triglycerides below 800 mg/dL.
The calculator does not reverse-engineer or replace your laboratory's LDL method. It uses the LDL number exactly as entered, then labels estimated remnant cholesterol as method-dependent.
Fasting status
Nonfasting panels can still be useful
Triglyceride context is the main reason fasting status matters here.
Current cardiovascular practice often accepts a fasting or nonfasting lipid panel for initial assessment. Fasting testing remains important in selected situations, including severe triglyceride elevation, suspected genetic lipid disorders, metabolic syndrome evaluation, and treatment follow-up.
The ACC Lipid Manager recommends a repeat fasting lipid profile when nonfasting triglycerides exceed 400 mg/dL. Triglycerides at or above 500 mg/dL are severe and require prompt clinical follow-up because pancreatitis risk becomes a concern, especially at 1,000 mg/dL or higher.
2026 guideline context
Your target depends on your risk
The same LDL value can mean different next steps for different people.
The 2026 ACC/AHA dyslipidemia guideline brought back LDL-C and non-HDL-C treatment goals. Examples include LDL-C below 100 mg/dL for borderline or intermediate primary-prevention risk, below 70 mg/dL for high risk, and below 55 mg/dL with non-HDL-C below 85 mg/dL for people with ASCVD at very high risk.
Those are risk-specific clinical goals, not general result bands. Age, diabetes, chronic kidney disease, known cardiovascular disease, family history, smoking, blood pressure, coronary calcium, and other factors change the interpretation. This calculator therefore shows arithmetic without assigning you to a treatment group.
What a standard panel does not show
The 2026 guideline recommends measuring lipoprotein(a), or Lp(a), at least once in adulthood. ApoB can be useful when triglycerides are elevated, diabetes or cardiovascular-kidney-metabolic syndrome is present, known cardiovascular disease exists, or LDL-C and non-HDL-C appear controlled but residual particle-related risk is a concern.

What does this calculator calculate?
It calculates non-HDL cholesterol, estimated remnant cholesterol, total-to-HDL, LDL-to-HDL, and TG-to-HDL in both mg/dL and mmol/L conventions.
Is non-HDL cholesterol directly measured?
No. It is calculated as total cholesterol minus HDL cholesterol.
What does non-HDL include?
It includes cholesterol carried outside HDL, including LDL and other apoB-containing lipoproteins.
Is estimated remnant cholesterol a lab measurement?
No. This tool estimates it as total cholesterol minus HDL-C minus LDL-C. Direct remnant assays and research methods are different.
Why can remnant cholesterol be negative?
A negative result can arise from rounding, assay differences, direct LDL methodology, transcription mistakes, or values from different draws. The calculator marks it not interpretable.
Why must all values come from the same draw?
The subtraction formulas assume all four values describe one blood sample. Mixing dates creates a synthetic result.
Is total cholesterol a treatment target?
The American Heart Association says total cholesterol should not be interpreted alone. Current clinical goals focus on LDL-C and non-HDL-C according to risk.
Is HDL a treatment target?
No. AHA states that HDL is not a stand-alone treatment target, although it remains part of overall risk assessment.
Does a high HDL cancel a high LDL?
No. HDL should not be used to erase or offset LDL-related risk.
Is the total-to-HDL ratio a diagnosis?
No. It is descriptive arithmetic and does not diagnose cardiovascular disease or determine a treatment plan by itself.
Is the LDL-to-HDL ratio a treatment target?
Not in the 2026 ACC/AHA guideline. LDL-C and non-HDL-C goals are selected according to overall cardiovascular risk.
Why are there two TG-to-HDL ratios?
Triglycerides and HDL use different mg/dL-to-mmol/L conversion factors, so the numeric ratio changes with the unit convention.
Can TG/HDL diagnose insulin resistance?
No. It has been studied as a marker, but performance varies by population, sex, metabolic context, fasting status, and unit convention.
Does calculated LDL mean Friedewald was used?
Not necessarily. A lab may use Friedewald, Martin-Hopkins, Sampson-NIH, or another validated method. Check the report or ask the laboratory.
Can I use a nonfasting lipid panel?
Often yes for general risk assessment. Fasting follow-up is important in selected situations, especially when triglycerides are very high.
What if nonfasting triglycerides are above 400 mg/dL?
ACC educational guidance recommends repeating a fasting lipid profile to assess fasting triglycerides and baseline LDL-C.
What if triglycerides are 500 mg/dL or higher?
That is severe hypertriglyceridemia in current ACC guidance. Contact a healthcare professional promptly because pancreatitis risk becomes an important concern.
What if LDL-C is 190 mg/dL or higher?
Current guidance treats LDL-C at or above 190 mg/dL as a level requiring clinical evaluation, including possible genetic and secondary causes. Discuss it promptly with a healthcare professional.
What are ApoB and Lp(a)?
ApoB reflects the number of atherogenic lipoprotein particles. Lp(a) is a largely inherited risk marker that the 2026 guideline recommends measuring at least once in adulthood.
