Why waist size matters more than total weight
Two people can have the same body weight and BMI while carrying very different amounts of health risk, depending on where their fat is stored. Fat stored around the midsection, called visceral fat, surrounds internal organs like the liver, pancreas, and intestines. Fat stored elsewhere, like the hips or thighs, called subcutaneous fat, appears to carry less metabolic risk.
Visceral fat is metabolically active tissue. It releases fatty acids and inflammatory signals directly into the bloodstream that feeds the liver, which is one reason waist circumference correlates more closely with insulin resistance and cardiovascular risk than total body weight alone.
How waist circumference fits into metabolic syndrome
Waist circumference is one of five criteria used by the National Cholesterol Education Program (NCEP ATP III) and the American Heart Association to evaluate metabolic syndrome, alongside triglycerides, HDL cholesterol, blood pressure, and fasting glucose. Having three or more of the five outside a healthy range is generally used to define metabolic syndrome.

Meeting three or more of these five criteria is generally used to define metabolic syndrome. Waist circumference is often the earliest of the five to shift, sometimes moving before blood work changes at all.
How to measure your waist circumference correctly
Measurement technique changes the number more than people expect. The standard method used in research and clinical settings:
- Stand up straight with your abdomen relaxed, not sucked in.
- Find the top of your hip bone (the iliac crest) on each side.
- Wrap a flexible tape measure around your bare abdomen at that level, roughly at the level of your belly button for most people.
- Breathe out normally, then read the measurement. Do not pull the tape tight enough to compress skin.
- Take the measurement twice and average the two if they differ.
Measuring over clothing, measuring at the narrowest point of the torso instead of at the hip bones, or measuring after inhaling can all shift the number by an inch or more.
Waist-to-height ratio: a simpler alternative
Some researchers argue that a single waist circumference cutoff doesn't account for differences in height, and propose waist-to-height ratio instead. The general guideline: keep your waist circumference below half your height. A person who is 68 inches tall would aim to keep their waist under 34 inches, using this method.
Waist-to-height ratio has shown a strong association with cardiometabolic risk across different populations and body types in research, in part because it naturally scales with a person's frame instead of using one fixed number for everyone.
Why belly fat and insulin resistance move together
Excess visceral fat is closely linked to insulin resistance, the state where cells respond less effectively to insulin and the pancreas has to work harder to manage blood sugar. The relationship tends to run in both directions: visceral fat contributes to insulin resistance, and insulin resistance can promote further fat storage around the midsection.

Beyond genetics, factors that are more within a person's influence include overall caloric balance, added sugar and refined carbohydrate intake, alcohol consumption, sleep quality, chronic stress and cortisol patterns, and physical activity, particularly resistance training and activities that support overall metabolic health rather than spot-reduction, which does not have strong evidence behind it.

No. Core exercises can strengthen the muscles underneath, but they don't selectively burn the fat covering that area. Fat loss happens across the body based on overall energy balance, genetics, and hormonal factors, not the specific muscles being exercised.
