A normal liver panel feels reassuring. But normal enzymes don't rule out fatty liver, and here's the biology behind why.

Quick answer: Yes. Liver enzymes like ALT and AST primarily reflect active liver cell injury, not the presence of fat itself. Research shows a meaningful portion of people with confirmed fatty liver on imaging have completely normal liver enzyme levels. Imaging, such as ultrasound or elastography, is more reliable for initial detection than blood work alone.
What liver enzymes actually measure
ALT and AST are enzymes that live inside liver cells. When liver cells are damaged or inflamed, these enzymes leak into the bloodstream, and blood levels rise. This makes ALT and AST good indicators of active liver cell injury or inflammation. But fat can accumulate inside liver cells without necessarily causing enough active injury to meaningfully elevate these enzymes, especially in earlier stages.
Why fat and inflammation aren't the same thing
MASLD exists on a spectrum. At one end, simple steatosis involves fat accumulation in the liver without significant ongoing inflammation or cell damage. In this stage, liver enzymes can remain within normal range even though imaging would clearly show fat present. Further along the spectrum, MASH (metabolic dysfunction-associated steatohepatitis) involves active inflammation and cell injury alongside the fat, and this is where liver enzymes are more likely to rise.
This means a normal ALT and AST can reflect either a genuinely healthy liver or early-stage fatty liver that hasn't yet progressed to active inflammation. Blood work alone can't distinguish between these two possibilities.
What actually detects fat in the liver
Imaging is more reliable for the initial question of whether fat is present at all:
- Abdominal ultrasound can detect fat in the liver and is often the first-line imaging test, though it's less sensitive for milder cases.
- Elastography (FibroScan) can estimate both the amount of fat present and the degree of liver stiffness, which relates to scarring or fibrosis.
- MRI-based methods offer the most precise fat quantification but are used less often due to cost and availability.
Who should consider screening despite normal enzymes
Given that enzymes can miss early fatty liver, screening decisions are usually based on your broader risk profile rather than liver enzymes alone. Relevant risk factors include elevated waist circumference, insulin resistance or type 2 diabetes, elevated triglycerides, low HDL, and elevated blood pressure, the same factors used in the MASLD diagnostic framework itself. If several of these apply to you, it's reasonable to ask your healthcare provider whether liver imaging makes sense, even with a clean liver panel.
