Where vitamin B12 comes from
Natural food sources include fish, meat, poultry, eggs, milk, and other dairy products. Plant foods do not naturally contain B12 unless they are fortified. Fortified nutritional yeast, plant milks, breakfast cereals, and meat alternatives can provide B12, but the amount and serving size vary.
A vegetarian who regularly eats eggs and dairy may receive meaningful B12, while a vegan depends on reliable fortified foods or supplements. Labels matter more than assumptions. Fermented foods, seaweed, or unfortified nutritional yeast should not be treated as dependable B12 sources without evidence on the specific product.
How metformin changes the equation
Metformin is associated with lower B12 absorption and lower circulating levels in some people. Risk tends to rise with higher doses and longer duration. Diet affects input, while metformin may affect absorption. When both point in the same direction, monitoring becomes more important.
This does not mean someone must abandon a plant-forward diet or stop metformin. It means the plan needs a reliable B12 source, an honest review of risk factors, and appropriate laboratory follow-up.
What current guidance says
ADA 2026 recommends periodic B12 assessment during long-term metformin use, especially with anemia or peripheral neuropathy. Its prevention guidance states that people taking metformin for more than four years or otherwise at risk, including those following a vegan diet, should be monitored annually.
The MHRA similarly advises testing when deficiency is suspected and considering periodic monitoring when risk factors are present. A vegetarian diet is not one uniform category, so clinicians should ask what foods and supplements are actually used.
How to audit B12 intake
List every fortified food used in a normal week.
Read the serving size and amount of B12 per serving.
Check whether intake is daily and dependable or occasional.
Include multivitamins, B-complex products, protein powders, and energy products.
Record the B12 form, dose, and frequency of supplements.
Bring the list to a clinician or registered dietitian rather than guessing from the diet label alone.
Symptoms are not a nutrition score
A person can have low intake without obvious symptoms for years because the body stores B12. Another person may have neurologic symptoms before anemia appears. Fatigue, numbness, balance changes, tongue soreness, and cognitive changes should be evaluated rather than treated as proof that the diet failed.
The same symptoms may come from iron deficiency, folate deficiency, thyroid disease, diabetes-related neuropathy, inadequate calories or protein, sleep problems, medication effects, or other conditions. Plant-based eaters deserve the same complete differential diagnosis as anyone else.
Choosing a B12 source without falling for hype
Fortified foods and supplements can both be effective sources. NIH reports no evidence that supplement absorption differs by form and no efficacy difference between oral and sublingual products in available evidence. The choice should emphasize a reliable amount, consistency, product quality, dietary suitability, and a monitoring plan.
A supplement should not be marketed as proof that a diet is complete. Protein, iron, iodine, calcium, vitamin D, omega-3 fats, and total energy may also require attention depending on the dietary pattern. A registered dietitian can review the whole pattern without treating B12 as the only question.
