Metformin and Vitamin B12 Deficiency: What Long-Term Users Need to Know

Metformin is effective and widely used, but long-term treatment can lower vitamin B12 in some people. Here is how to recognize risk without stopping a useful medicine.

Why metformin and vitamin B12 are connected

Metformin remains a foundational glucose-lowering medicine because it is effective, familiar, and useful for many people. One recognized long-term issue is reduced vitamin B12. The UK medicines regulator now describes this as a common adverse effect, with risk increasing as dose and treatment duration rise.

Vitamin B12 supports normal nerve function, red blood cell formation, and DNA synthesis. A low level may show up as anemia, neurologic changes, or both. Neurologic symptoms can occur without anemia, which is why a normal blood count does not close the question when symptoms or risk factors point elsewhere.

What long-term studies found

In a randomized placebo-controlled trial published in 2010, long-term metformin treatment increased the risk of biochemical B12 deficiency and raised homocysteine compared with placebo. This helped establish that the relationship is more than a collection of isolated case reports.

The Diabetes Prevention Program Outcomes Study followed people for many years. Low and borderline-low B12 were more common among participants assigned to metformin, and each additional year of metformin exposure was associated with higher odds of deficiency. The study also observed more neuropathy among metformin users with low B12, but that association did not prove B12 deficiency caused every neuropathy symptom.

A systematic review and meta-analysis reached the same broad conclusion: metformin use is associated with lower B12 measures. Studies differ in dose, population, duration, definitions, and laboratory methods, so the evidence supports attention and testing rather than predicting one fixed timeline for everyone.

Who may have higher risk

People taking metformin for several years, especially beyond four to five years.

People taking higher daily doses, including doses around 1,500 mg per day or more.

People with anemia, new peripheral neuropathy, or neurologic symptoms.

Vegans and some vegetarians who obtain little B12 from fortified foods or supplements.

People with gastric or small-bowel surgery, inflammatory bowel disease, celiac disease, or other absorption problems.

Older adults and people taking acid-suppressing medicine or other drugs associated with reduced B12.

People with a previous low or borderline result.

Risk factors can stack. A person taking metformin for six years who also eats a vegan diet and uses a proton-pump inhibitor has a different risk profile from someone who recently started metformin and regularly eats B12-rich foods. Monitoring should reflect that context.

Symptoms worth evaluating

Possible symptoms include unusual fatigue, weakness, a sore or red tongue, pale or yellow-tinged skin, pins and needles, numbness, problems with balance, memory or thinking changes, and mood changes. These symptoms are not specific to B12 deficiency. Diabetes, thyroid disease, iron or folate deficiency, kidney disease, sleep problems, medicine effects, and many other conditions can look similar.

Do not wait for anemia: NICE advises that the absence of anemia or large red blood cells does not rule out B12 deficiency. Neurologic symptoms deserve timely assessment even when a complete blood count appears normal.

What current monitoring guidance says

The 2026 American Diabetes Association recommends considering periodic B12 assessment in people receiving long-term metformin, especially when anemia or peripheral neuropathy is present. Its prevention guidance points to higher dose, four to five years of use, chronic kidney disease, and other B12 risk factors. It states that people taking metformin for more than four years or otherwise at risk, including those following a vegan diet or with gastric or small-bowel surgery, should be monitored annually.

The MHRA advises testing when deficiency is suspected and considering periodic monitoring for people with risk factors. These statements do not create one universal schedule for every person. The appropriate interval depends on duration, dose, symptoms, prior results, diet, gastrointestinal history, kidney function, and the clinician responsible for care.

How B12 deficiency is tested

A total serum or plasma B12 level is a common first test. NICE also allows active B12 as an initial test. A very low result can make deficiency likely, while a clearly adequate result may make it less likely. The difficult area is the middle, where symptoms and risk factors matter.

NICE uses total B12 below 180 pg/mL as a confirmed result and 180 through 350 pg/mL as indeterminate, while noting that local laboratory ranges apply. NIH describes roughly 150 through 399 pg/mL as a range in which methylmalonic acid, or MMA, can help confirm deficiency. MMA is sensitive but may rise with reduced kidney function and age. Homocysteine can also rise when B12 is low, but folate status and kidney function make it less specific.

Testing is best obtained before starting replacement when it is safe to wait, because over-the-counter B12 can raise the blood level and complicate interpretation. If severe neurologic symptoms are present, clinical care should not be delayed simply to create a cleaner test result.

What happens after a low or borderline result

The next step is not simply choosing the largest supplement dose. A clinician may review symptoms, complete blood count, folate, kidney function, diet, medicines, gastrointestinal history, and confirmatory tests. The cause matters because dietary intake, medicine-related absorption, autoimmune disease, and gastrointestinal conditions may require different follow-up.

Treatment may use oral or injected B12 depending on the cause, severity, symptoms, absorption, access, and patient preference. NICE specifically allows oral or intramuscular replacement for medicine-induced deficiency based on clinical judgment. Metformin can often continue while B12 is corrected, but only the treating clinician should make medication decisions.

List symptoms, when they began, and whether they are changing.

Bring prior B12 results, complete blood counts, and kidney-function results if available.
