Serum B12, MMA, and Homocysteine: What Each Test Measures

Total B12 measures what is circulating. MMA and homocysteine look for metabolic consequences, but each has limitations that matter in real interpretation.

Quick answer: Total serum or plasma B12 is a common first-line test. MMA is a sensitive functional marker that can help when B12 is borderline, but kidney impairment and age may raise it. Homocysteine can also rise with B12 deficiency, yet folate deficiency and kidney function make it less specific. No result should be interpreted in isolation.
Total serum or plasma B12
This test measures B12 circulating in blood. It is widely available and often the starting point because a very low value can support deficiency and a clearly adequate value can make deficiency less likely.
The test has a gray zone. It measures B12 carried on several proteins and does not directly measure whether every cell has adequate functional B12. Assay methods and reference ranges vary. Recent supplements can raise the number before the clinical question is resolved.
Active B12, also called holotranscobalamin
Active B12 measures the portion bound to transcobalamin that is available for cellular uptake. NICE allows total B12 or active B12 as the initial test in many people. The choice often depends on local laboratory availability and clinical practice.
Active B12 still needs context. NICE describes below 25 pmol/L as confirmed deficiency, 25 through 70 pmol/L as indeterminate, and above 70 pmol/L as making deficiency less likely, while directing clinicians to use local ranges where appropriate.
Methylmalonic acid, or MMA
B12 is needed to convert methylmalonyl-CoA to succinyl-CoA. When functional B12 is inadequate, MMA can accumulate. NIH calls MMA the most sensitive marker of B12 status and notes that levels above about 0.271 micromol/L can suggest deficiency, although laboratories use their own ranges.
Reduced kidney function can raise MMA because clearance changes, and values may increase with age. A high MMA is therefore strongest when it fits the B12 result, symptoms, risk factors, and kidney context. NICE recommends considering it when symptoms are present and the initial B12 result is indeterminate.
Homocysteine
B12 participates in the conversion of homocysteine to methionine. When B12 is inadequate, homocysteine may rise. However, folate status, kidney function, age, and other factors also affect it.
NIH describes homocysteine as having poor specificity for B12 deficiency. NICE tells clinicians to use the laboratory range and consider folate deficiency. It is a supporting marker, not a standalone diagnosis.
How the tests can work together
Start with symptoms, risk factors, metformin duration and dose, diet, medicines, and gastrointestinal history.
Use total or active B12 as the initial laboratory measure when appropriate.
If the initial result is clearly low, evaluate cause and treatment rather than waiting for every possible marker.
If the result is indeterminate and symptoms are present, consider MMA or another clinician-selected test.
Interpret MMA with kidney function and homocysteine with folate and kidney function.
Connect follow-up testing to symptoms, cause, route of replacement, and adherence.
What a complete blood count adds
A complete blood count can identify anemia and changes such as increased mean corpuscular volume. It can support the diagnosis and reveal other hematologic problems. It cannot rule out neurologic B12 deficiency when normal.
Iron deficiency can sometimes mask macrocytosis, and mixed deficiencies can produce a less textbook pattern. Clinicians interpret the blood count beside the B12 question, not as a substitute for it.
Testing before supplements
NICE advises obtaining diagnostic blood before B12 replacement when possible because supplements can change the values. Bring the bottle or a photo of the Supplement Facts panel, including multivitamins, energy products, fortified powders, and sublingual B12.
When neurologic symptoms are severe or rapidly worsening, a clinician may decide that treatment should not wait. The balance between clean testing and timely care is a clinical decision.

Which B12 test is best?
There is no single best test in every situation. Total or active B12 often starts the evaluation, with MMA useful when the result is indeterminate.
Can kidney disease cause a high MMA?
Yes. Reduced kidney function can elevate MMA, so the result should be interpreted with kidney measures and the full clinical picture.
Can folate deficiency raise homocysteine?
Yes. That is one reason homocysteine is less specific for B12 deficiency.
Can my blood count be normal if B12 is low?
Yes. Neurologic manifestations can occur without anemia or macrocytosis.
Should I stop supplements before testing?
Do not make a blanket change on your own. Tell the clinician what you take and ask how to prepare, especially if symptoms are significant.
